Prior to your first visit I will send an intake questionnaire, consent for treatment, privacy and procedure policies, and credit card information. I ask that you complete these prior to coming in so we start your session on time.

Insurance:
Currently, I am not on insurance panels.  Some insurance policies will provide partial reimbursement for psychotherapy sessions with "out-of-network providers". 

I can provide you with a Superbill that you may submit to your insurance company for potential reimbursement. 

 

Whether or not your insurance plan will reimburse you for sessions depends upon your policy and the specific coverage.

 

Please call your insurance company for more information regarding coverage.

48-Hour Cancellation Policy:
I maintain a 48 hour cancellation/rescheduling fee policy.  You will be responsible for the entire session fee ($200.00) if cancellation/rescheduling is less than 48 hours.

 

Late Arrival:

Appointments are 50 minutes in length and begin and end promptly.  Late arrivals will generally be accommodated, but the session will still end at the scheduled time and you will be charged the full fee.
